What You'll Do

Complete Start of Care assessments, admissions, recertifications, discharges, and skilled nursing visits for home health patients
Perform comprehensive patient assessments and coordinate care plans with physicians and the interdisciplinary team
Complete accurate OASIS documentation and maintain compliance with Medicare and home health regulations
Monitor patient progress and identify changes in condition that require intervention or follow-up care
Educate residents and families regarding treatment plans, medications, and disease management
Follow all home health regulations, safety, infection control, documentation, and mandatory reporting policies

What You'll Need

Current Florida Registered Nurse License (required)
Previous home health nursing experience (required)
OASIS documentation experience and knowledge of home health regulations (required)
Current CPR certification (required)
Strong assessment, communication, and clinical documentation skills
Knowledge of Medicare regulations and ACHC standards
